Acknowledgement of Vaccination Requirements and Cancellation Policy

Please read and acknowledge the following vaccination requirements and cancellation policy for the services you are requesting:

For the protection of your pet, other pet clients and our staff, it is the policy of Beau Tye’s Pet Spa, LLC that all pet clients be current on the following vaccines for all services:

1) Rabies (within previous 12 months)

2) Distemper (DHPP) (within previous 12 months or 3 years, which ever your veterinarian provides)

3) Veterinarian appropriate bordatella/kennel cough vaccine (6 month or annual)

Puppy clients must be current on age-appropriate vaccines and also have the appropriate bordatella/kennel cough vaccine.

Acknowledgement of Cancellation Policy

Because we book by appointments, should you need to cancel your appointment for any reason, we require that clients call to cancel at least 24 hours in advance of your pets scheduled appointment date. Otherwise, a $30.00 rebooking fee charge will be applied.

By checking this box I acknowledge the cancellation policy and understand that if I do not cancel any standing appointment within 24 hours that I must pay a $30.00 rebooking fee for each dog that is scheduled.
